JOIN US FOR OPEN HOUSE:
On January 28, 2012 Our Lady of Perpetual Help School invites you to visit our Great School from 6:30 p.m. until 7:45 p.m. Come see all the great things OLPH School has to offer.
OPEN HOUSE #2 will be held on Friday, February 3rd from 10 am until 12 noon. Children are invited to be an “OLPH Student for the Day.” Children will take a tour of the school and then spend time in the classroom with an OLPH Student. Parents will take a tour of the school and will have the opportunity to ask questions and talk with OLPH Parents, students & teachers.

We are currently accepting applications for new students in all grades at OLPH. If you would like to begin the application process during the month of July (while the school office is closed) please email Mrs. Davis (ddavis@ourladyschool.com) to make an appointment. Please proceed to the admission tab for instructions and to download our application.
OLPH is a Catholic school with 100% acceptance to Catholic high schools. Our students are very involved in a wide variety of extracurricular activities, sports and clubs. These activities make our students well rounded and ready for the challenges of the most elite high schools and colleges.
Admission Process
The OLPH Application process is “on-going”.
Step One: Complete and submit the OLPH Contact Form. Once we have received the Contact Form we will schedule a School Tour, Student Testing and a Family Interview. Step Two: Submission of Application Documents and Testing Fee. The following documents are required before full acceptance to OLPH School can be made:
- Download the OLPH Application Form. Click on the OLPH Application link to print.
- Birth Certificate issued from the state in its original form
- Testing Fee – $50.00 nonrefundable (must be paid on day of testing)
- Baptismal Certificate – original
- Verification of OLPH Parish Registration / Parish Envelope & number
- Religious Education verification if applicable
- First Communion Certificate if applicable
- Current Report Card for Grades 1 – 7
- Current Standardized Test Scores for grades 2 – 7
- School of Transfer address and phone number
- Up to Date Immunization Record (yellow card)
- Documentation of recent doctor’s exam
- Health Questionnaire
- Family Photo
Step Three: While your child is taking the entrance test you will go on a School Tour. During the school tour you will be able to visit the classrooms and meet our teachers. We will answer any questions you may have about Our Lady of Perpetual Help School. You will be able to see first hand all that is unique about our school.
Step Four: Your child will take an entrance test to determine their individual achievement level. Each test is age / grade appropriate. In the case of the Kindergarten students, the test measures readiness for our all-day Kindergarten program. There is a testing fee of $50.00 which will be collected on the day of the test. This fee will be put toward your tuition/fees upon acceptance to OLPH School.
Step Five: Notification of Acceptance will be sent within two weeks of testing and interview provided we have received all of the documents from Step Two.
Criteria for Acceptance to OLPH School
- A desire to have a Catholic Education with an emphasis on educating the “whole child”
- Successful placement on the entrance exam
- Student must meet the OLPH school age requirements. (Kindergarten must turn 5 years by the fall of their Kindergarten year)
- Parents agree to the Statement of Commitments found on the Statement of Parent Commitment Form
