Admission Process
The OLPH Application process is “on-going”. Contact Forms can be delivered to the school office any time throughout the school year.
Step One: Complete and submit the OLPH Contact Form. Once we have received the Contact Form we will schedule a School Tour and Family Interview.
Step Two: Submission of Application Documents and Application Fee. The following documents are required before full acceptance to OLPH School can be made:
- OLPH Application Form
- Birth Certificate issued from the state in its original form
- Social Security number – optional
- Application Fee – $42.00 nonrefundable
- Baptismal Certificate – original
- Verification of OLPH Parish Registration / Parish Envelope & number
- Religious Education verification if applicable
- First Communion Certificate if applicable
- Current Report Card for Grades 1 – 7
- Current Standardized Test Scores for grades 2 – 7
- School of Transfer address and phone number
- Up to Date Immunization Record (yellow card)
- Family Photo
Step Three: During the School Tour you and your child will visit the classrooms and meet our teachers. We will answer any questions you may have about Our Lady of Perpetual Help School. You will be able to see first hand all that is unique about our school.
Step Four: Your child will take an entrance test to determine their individual achievement level. Each test is age / grade appropriate. In the case of the Kindergarten students, the test measures readiness for our all-day Kindergarten program.
Step Five: While the student is taking the entrance exam, one or both parents will interview with a member of the OLPH Administrative staff.
Step Six: Notification of Acceptance will be sent within two weeks of testing and interview provided we have received all of the documents from Step Two.
Criteria for Acceptance to OLPH School
- A desire to have a Catholic Education
- Successful placement on the entrance exam
- Student must meet the OLPH school age requirements. (Kindergarten must turn 5 years by the fall of their Kindergarten year)
- Parents agree to the Statement of Commitments found on the Statement of Parent Commitment Form
